Online Booking
Booking appointments online is simple and easy using the "Book Online" button below. If it is your first time scheduling an appointment online with me, you will be taken through the steps of setting up a patient portal account as you reserve your appointment time.

The patient portal is also where you'll find the new patient forms. It is preferable that you fill out these forms before your appointment time, otherwise some of your appointment may need to be used to complete the forms. You may fill out the forms online, print them out and bring the completed copies to your appointment, or arrive to your appointment 15 minutes early if you would like us to provide printed copies for you to fill out in the office.
If you don't see a suitable appointment time online, don't hesitate to give me a call, and I'd be happy to check for openings that may not show up on the online system.

Rescheduling or Canceling Appointments
Appointments can be rescheduled or canceled online either through the patient portal or the link provided in your appointment confirmation email. You are also welcome to call for assistance in finding an appointment that would work better for you.
If you need to reschedule or cancel, please do your best to do so at least 24 hours before your appointment. If your appointment is rescheduled or canceled with less than 24 hour notice, or if you miss your appointment, you may be charged a $50 fee. This fee does not apply for cancellations or missed appointments due to Covid exposure or symptoms.
